EP-664 Remnant of stomach strangulation post Roux-en-y gastric bypass and hiatal hernia repair for obesity

Abstract Introduction and Background Herniation of the remnant stomach post Roux-en-gastric bypass hiatal hernia repair is a very rare complication that require high index suspicion to diagnose immediate surgical intervention. In literature, there were reported cases herniation gastric pouch Roux limb but not hence we report this unusual case. Case A 66-year-old lady was admitted with dysphagia, vomiting, epigastric retrosternal chest pain. She has had laparoscopic Roux-en-y 8 years before admission complicated recurrence which repaired one year prior presentation. CT scan abdomen showed both neo-stomach up in posterior mediastinum through hiatus. Patient consented for on next day however her condition deteriorated over night taken theatre underwent emergency converted open resection strangulated ischemic preservation limb. Conclusion This uncommon event following repair, prompt diagnosis early intervention imperative avoid catastrophic complications.
